Ovarian remnant syndrome is an uncommon condition in which women who have had both ovaries removed in the past have residual ovarian tissue in the body. The remaining ovarian tissue can grow, cause pain, or even form cysts that may hemorrhage. Ovarian tissue remnants can re-implant anywhere in the pelvis.
Ovarian remnant syndrome can occur because the initial surgery to remove the ovaries was extremely difficult and a piece of the ovary was left behind. In women with endometriosis or a lot of scar tissue in the abdomen for other surgeries, removing the ovary safely can be a challenge.
Women with ovarian remnant syndrome may develop new or worsening chronic pelvic pain that prompts further evaluation. Imaging (MRI and ultrasound) in conjunction with specific hormone tests can help in the diagnosis. Ultimately, if ovarian remnant syndrome is suspected, surgery must be performed to investigate the pelvis, biopsy suspicious tissue, and remove the remnant. These surgeries can be very complex and the surgical plan needs to be individualized.
